Area of travel for travel insurance

When purchasing travel insurance do I need to include any countries through which I will transit? We are visiting Nicaragua in November and I need to get connecting flights in the USA. The flight has been booked as a single ticket all through one airline (United). Do I need Worldwide Insurance excluding or including USA? We probably won't even be leaving the airport but it adds a lot to the cost of the insurance by including the USA.

Ask the insurer...
For most countries I would say that you do not need to be concerned about countries where you change 'planes, since technically you do not enter them but merely wait in the transit area. The USA, however, makes transit passengers pass Immigration so that technically they do enter the country before proceeding to their onward flights.0 -
If you develop appendicitis on the plane heading to the USA you're going to be treated in a very expensive hospital there.
