First Direct Switching

Hi All, I applied to switch via the best moneysaving/cashback route to switch to First Direct. I received all the account information and debit card. I believe all I had to do was contact them for appropriate switch date.
Sadly the application was made last May 2015 and due to family matters the paperwork got filed away and I didnt do anything with it. Would anyone know the following:
Is the account still likely to be 'live'?
Due to the delay will I lose the cashback deal?
