Terminology to help with a credit report complaint

Hi,
I'm wondering if someone would be able to give me a bit of help with some wording on a complaint.
Basically I recently viewed my credit report and noticed a default being reported by British Gas for unpaid gas and electricity every month from March 2012.
They are also reporting the accounts twice so it shows us as four defaults.
I moved out in April 2012, provided a final meter reading and forwarding address. Never received any demand for payment and thought all was well.
From the account being marked as OK in April it went straight to being reported more than six payments in arrears in March and defaulting a year or so later.
I've rang them numerous times and raised a complaint but no one has rung me back yet so I'm emailing them instead with the issues.
The first person I spoke to said they made a mistake when closing the account because by now it should be with a debt agency but the debt is still with British Gas so they need to manually create another bill.
Basically my complaint will be account closure not completed and affected my credit score for the last 4 years. It's stopping me getting a mortage as as I work for a financial institution it could affect my employment.
Bad record keeping? Something along the lines of that?

Only you can see your credit score, what this has done has added entries onto your credit file that the bill has been unpaid. Whats the date for the defaults, is it 2013 ?0

Date of default on both accounts is 17/08/20130
