AI Scheme Letters?

Hi Folks,
I'm after a little help as I have no idea on this.
Basically I have the initial letter tell me that I may be entitled to compensation and I must claim before 18 March 2016.
I also have the "Card Security Product: Compensation Claim Form" with the section I need to complete to say why I want to be considered.
So basically I have no idea what product/card this was for, whether I'm entitled, whether it's worth doing or its even legit? Also the card was not lost or stolen.
I looked at the templates here: http://www.moneysavingexpert.com/credit-cards/Sentinel-Affinion-card-protection#write but still not sure what the best course of action is.
Any help most appreciated, thank you in advanced!
