Nationwide PPI Claim

Hello
After making an initial enquiry, Nationwide have sent me a letter regarding PPI. The letter is quite brief but basically says, regarding PPI:
Mortgage - Not Held
Credit Card - Not Held
Personal Loan - Held
The issue I have is that over the years I have had several Personal Loans with Nationwide and I don't know which one they are referring to. I don't have any paperwork.
The letter they sent says that if I wish to go ahead and make a complaint I can fill in a questionnaire on their website. I started to do this but at one point one of the mandatory fields asks what my job was when I took out the PPI policy. I've had a number of jobs in the last few years so don't know which one applies as I'm not sure of the dates...
Has anyone else been through this with Nationwide without the required details?
