Salary Sacrifice and P11d's

Hi All
I was hoping that you could help me with regards to P11d's and salary sacrifice. I have trolled through the millions of documents you can find online and on the HMRC website, but I still feel its all a little bit too grey for my liking!.
Could anyone tell me what salary sacrifices need to be logged on a P11d and whether or not they have NIC Liability on them? And if they do need logging what section of the P11d do you fill in?
We are looking at Cycle to Work Scheme, Childcare Vouchers, Car Leasing, Electrical Goods and Car Parking.
Also is there a limit to the amount you can sacrifice? Could you sacrifice all your wage up to mimimum wage limits?
