ESA medical form

I'm due for a review soon and spoke to someone from DWP and they told me they'll send me a medical review form. Does anyone know what this form is exactly?

Is it a lengthy form where I fill in how much disability effects me and my daily life or or it just a short form where I confirm that my illness is still effecting my ability to work or what?

Anyone had this letter before?

    It is a long form, known as the ESA50.

    Why do I have to fill out this form again?! The way my disability effects me and all the trouble it brings me is already on their system... I filled out this ESA50 form 12 months ago, why do I need to fill it out again?

    Because you wish for the govt to continue providing you with an income?

    If your condition has not changed then write/ print the same as last time.

  • I must echo what Tomtontom says....all i do when i get my ESA50 is copy exactly what i wrote on the previous ones they sent (or as near as possible).

    Because the person who assessed you indicated that your condition may change over a period of 12 months. DWP needs to know if your condition has improved or worsened during that period.
    The effect a medical condition has on the individual can and does change over time.

    I must echo what Tomtontom says....all i do when i get my ESA50 is copy exactly what i wrote on the previous ones they sent (or as near as possible).

    To save time, I downloaded the PDF and filled it in using the tools, Meaning all I need to do is modify any changes and print then send....A lot of time and energy saved ;).

    Although am dreading my review this year.
