I need to fill this in by the 15th January but I am having severe difficulty.
Due to a severe blow to the head a few months ago My life has become unbearable.
I just cant put in to words how this has affected my life.
If my daughter fills this form in for me would that be ok as I just cant make any sense of it.

Yes, it's fine for your daughter to write it. Give as much detail as you can, and don't worry about repeating yourself. Also if you can include medical reports that will help get your claim dealt with promptly.0

Can I send copies of the medical reports or does it have to be originals and should the form be sent back signed for or just in the envelope they provide.0 -
you can send copies, I always have done.. make sure you copy the form and send it recorded delivery to avoid it getting lost.. best of luckMany thanks to all who contribute on MSE
