Should be no problem with a manager issuing a refund for you and it should not delay the refund either, it just means the agent needs to speak to their manager to authorise which can probably be done when you are on the phone.
As for readings I know BG ask the company they use for meter reads (who other suppliers also use) to visit on a quarterly basis, whether they come depends on if the company are in your area that quarter. Most suppliers terms and conditions ask that you provide an accurate reading every six months, albeit in the small print!0
