And mis-keying the name and address and accidentally entering exactly my name and (former) address? Any mathematicians in to work out the probability?

I had the same happen a couple of years ago for a magazine subscription and pet insurance and just checked my bank account today and have another magazine subscription taken out it in my name and address, I have done a bit of detective work and my name and address are correct but email is completely wrong and the phone number is similar but not correct, now going to be checking my bank accounts every day and wonder if I will see one for Pet insurance soon!!:(

You should name the pet insurance company. If it's always the same company, it points to a rogue employee in that company and the Police and the CEO of the company should get informed.
