Opening PDF document -toolbars missing

When I download my PDF MBNA credit card statement, doubleclick to open it, all the toolbars at top are missing.
There are no toolbars at all.
I have Adobe Reader 9.1 installed.
It doesn't do this with other PDF documents, so assume MBNA have customised the document to not display the toolbars.
The missing toolbars in another PDF document look like this:

How do I display the toolbars again, if possible ?
I have looked in settings, but can't see how.

Yes I have that one like you
I use Foxit Reader and have to move mouse over document then right click mouse and ask it in the drop down menu that appears to display the toolbar and menu bar.
All very tiresome and totally pointless.
Even when you save it and then re-open it reopens with the bars removed again.0 -
I also have some pdf documents that open from websites that are missing the toolbars. What I have found (Windows 7) is that if you move the mouse to near the bottom of the screen a menu appears which includes the Adobe icon on the far right ... click on that and the pdf then opens in the full blown version of Adobe Reader complete with toolbars. If the File ... Edit etc. menu is still missing from the top, clicking the Alt key makes it appear....
