Aviva help themselves to my bank account and then tell me later

The wife and I alternate our Aviva car insurance so we both keep our no claims discount despite owning one car.
The wife took out the latest policy in October and I paid the premium. She then gets a letter today saying "That further to our previous letters ( What letters!!!!) we have not received proof of the NCD.
This means from 04/12/2015 an additional premium of £**** will be debited from your account"
Well we got no letters, the online Aviva account covering letter does not ask for proof and we were sent no emails yet they inform me that last week they have helped themselves despite us both having NCD with Aviva for years.
This is on top of when I cancelled my old policy with them with just 2 weeks to go as we had sold the old car in late September so reducing their risk to zero where they charged me around £25 for the info.
Well Aviva what is your game? Have I given you permission to raid my bank account? Why are you telling me that you have sent us letters when you haven't? Why doesn't our Aviva online account show any request for proof on a NCD? Could it be that we are already insured with you so you should already know about it?
First time this has happened to us so I don't know what is different this year as we have never been asked to send in proof before.

I'm really sorry to see your post. I would like to look in to this for you and get this resolved.
Please could you send an email to social@aviva.co.uk with your full name, date of birth, postcode and policy numbers please, I will be able to look in to this for you.
When you email in, please could you also include your MSE username to help us link the email to your post the policy number / company where the No Claims Discount is coming from, we will be able to validate this on your policy and arrange a refund.
