Question about PPI

I have been having problems with claiming PPI with Lloyds bank.
I used to bank with Lloyds TSB, I contacted Lloyds about claiming PPI, they said that I would have to log it as a complaint and they have sent out forms for me to fill in asking me about the account numbers, repayments and repayment dates, but as it has been quite a long while ago, I no longer have the information they require, only the names of the companies.
Does anyone know if the company name would be enough for them to continue with my claim, another bank my friend used only needed company name and they searched the rest and are dealing with the claim at the moment. Thanks in advance.

If I have read your post right.
You only claim from Lloyds, they do not look for details from "other companies" you need to contact the other companies separately.

You could send a Sar request to Lloyds with cheque for £10, ask for ALL INFORMATION THEY HOLD ON YOU. Give them 40 days to respond, due to the time that has passed, it is possible there is no info there.

With the Lloyds form you have, personally, I would fill it in, put your Address, name and any other names you have been known by to see if they can find you.
You do not know the loan account numbers so put " I can't remember".
Put the reasons you have for mis sell and see what they come back with.
