Risk Assessments

My daughter is going on holiday early next year as part of the care plan paperwork I have to complete risk assessments and I have no idea where to start .
Does anyone know of any sites with templates or how to word it ?

Is she going alone?
Is it with a company that provides special care and support?
This is one such company and what they offer so perhaps you can base your wording using some of the services they offer?

have you asked them if they have a template?
What you're trying to do is consider what might go wrong, how bad it could be if it did, and what can be done to reduce the risk. I've seen a simple scoring system used at work.Signature removed for peace of mind0 -
If she needs a care plan for a holiday, it seems likely she has one for other parts of her life eg domiciliary care, respite care, care at school or nursery. I suggest trying to get copies of those risk assessments and then basing your risk assessment on them (both content and layout)
It might be worth saying that there'll be potentially two kinds of risk you'll be looking at
1) risks to the child. eg scalding, falling etc
2) risks presented to others while they support the child. eg if she doesn't have good control of her limbs and could therefore end up inadvertently hitting them
