Beneficial Finance Credit Card application Form PPI

Hi,
Im wondering if anyone on here can help. In 2002 I applied for a Beneficial Finance credit card which has long since closed. I didnt have any records relating to the application to check if I had PPI and I requested a copy of the application form from them. I have received a print out of a scan of the application form. However the quality is that bad I cannot see whether the tick in the payment protection box is a Yes or No. Has anybody got a copy of a application form from that time that can help me know which tick box is which and therefore if the application included PPI.

The bank will probably have a better copy, printing it out will have made it worse, try ringing them and asking, nothing to lose for that.
Do remember though a couple of points:
1) You could have cancelled the PPI or, conversely, you could have ticked no and later agreed to it on a phone call post-sale - simply having yes or no on the form isn't proof you paid or it was miss-sold
2) With CC PPI if you paid in full each month you could have PPI but would never have been charged anything
3) Do you have any statements showing you paid it? If they don't have a copy (did you do a £10 DSAR?) and you don't, you won't have any proof of anything being paid and there is a good chance they will have destroyed the records if it was closed more than 6 years agoSam Vimes' Boots Theory of Socioeconomic Unfairness:

Beneficial have been taken over by HSBC I cant post links but if you google. HFC(was one of the trading names of Beneficial) PPI you will get a phone number to call. All I had to give them was my name and address at the time and they told me over the phone that I did have PPI, got the ball rolling for my claim too.0
