Single status affidavit

I recently found out in my rush to complete student finance forms for my kids I clicked the wrong box on one of them and now need a single status affidavit, I went to the solicitors to be told they can't draw one up but if I write it they can sign it at a cost, but I have not idea what to put in it can anyone help me please.

Ignore that, when I looked more closely it was actually a declaration of marital status aimed at someone wanting to get married. It's not as obvious as I thought it might be, you may have to do an image search and see if there's something that fits the bill.
