New claim for loan and cards

I am certain that I had PPI on loans in the 80s and 90s and also had PPI on a TSB Trustcard when first taken out. I have no records or information on any of the loans, but having done a credit check have found out my Trustcard started in 1984.
I have rung LLoyds PPI line and explained the situation and they say just fill 2 forms out one for each claim.
Without the dates of when the loans where taken out, how can I fill the form in and if there is more than one loan with PPI on will I need those dates?

    Just give an approximation of the date if you are not sure. If you have such a clear memory of the sales that you can remember PPI being missold to you, I would not expect an approximate date to be a problem?
