My mother is 82 and going for a eye test in Boots
I know she qualifies for a free test.
But does she need to fill forms out before she goes
or will they have the relevant forms at the optician ?
Thank you

I'm not even sure there's a form to fill in.
If there is Boots will fill it in and all she will have to do is sign it.0
