Capital One ppi form question

Hello,
This is my very post and I am a little nervous (actually, very nervous) and I am hoping that someone will be able help me. I have several questions regarding ppi but in this post I will just focus on Capital One.
My questions are:
1) When I make a claim/compliant do I use the form on CO website or can I use the one from this site/ step three?
2) If I have to use the form from CO's website can I skip any of the questions? (ex. In SECTION C - PERSONAL CIRCUMSTANCES it says... you must provide evidence such as letter from tax office (HMRC*) showing that you were paying CLASS 2 or CLASS 4 NATIONAL INSURANCE.) I don't have any evidence from the Tax office, etc.
2b) Why would CO need this information?
I know these probably seem silly questions but this is a serious stumbling block for me.

Are you self employed or a director of a company, retired or not working when you took the PPI?.
If not, you do not need to fill the NI part in.0 -
Thank you for the quick reply. To be perfectly honest I am not sure, back when I applied for the card it was a mess and it is very complicated to explain, as in, I went from not employed, to student then back to not employed in a very short span of time .0
