I have had more than one loan with the same lender - do I need to fill a separate claim form in for each loan?

If your complaint reasons are the same for all the loans then you can use just the one form, just list all the loans you are complaining about0

Are you talking about a FOS Questionnaire?
You need a separate one for each loan as you need provide details of your personal circumstances at the time you took out each one (employment status, medical conditions, savings etc.). Most upholds are suitability related
If you are making allegations you need to provide specific detail for each sale individually. Just adding a handful of loan numbers to 1 questionnaire with a generic complaint point is not going to get you anywhere.0
