What is the best way to use a hand steamer?

I got one a while back for things like furniture to get rid of body sweat and the such and also for ironing clothes.
I tried it on a shirt and trousers and it worked fine but it didnt seem to work on a bedsheet, or pillowcases or at most got rid of the bigger creases and left the other ones just like a iron would.
I may not be using it right as tried holding it in front of the item hanging from door and as close as I can get or actually touching the fabrics but I notice the steam only comes out strong if I hold the device flat and upwards. else it sounds like a kettle boiling and not much steam comes out.
