I don't know the specifics of the particular card the OP has (in fact I don't think I can remember Amex doing 0% intro rates, but that's by the by), but Amex always seem to be the most 'old fashioned' to me when it comes to setting up DDs, requiring you to download and print a form from their website and then send it back to them (although at least you can upload it back rather than actually have to post it).
