Might be best to contact them but it seems you can change mid-term:

If there is a valid reason for cancelling your agreement, for example, if your mobility needs have changed and the car is no longer suitable, please call our Customer Services team on 0300 456 4566.

Every case is different but Motability are usually very understanding when someone's needs change due to medical reasons. I would be fairly confident they would let you change your car under these circumstances.
Its amazing how these banks can't even do simple calculations correctly..............
  • Mid contract I had to change from an auto to a wheelchair accesible vehicle.

Motability were amazing. Just one quick phone call to them for them to give me the go ahead.

OP I haven't been in your exact situation, but in my long experience of dealing with Motability I can tell you that they are amazingly helpful, and will do everything possible to ensure that you continue to have a vehicle that is suitable for you.
If you have a change in your medical situation, they will accept that as a valid change of circumstances, and allow you to change your vehicle.

If you are concerned, give them a ring and ask them, they'll be happy to explain what can happen if you are told you can't continue to drive your manual car.
Please don't worry tho. Motability will be as helpful as they can be.
I try not to get too stressed out on the forum. I won't argue, i'll just leave a thread if you don't like what I say. :)
  • As all have said, give them a call and I am sure they will be very understanding. They are a great company and out there to help people who need some help!
