I am trying to download Adobe (version2.0.0.46), it downloads to desktop as "flashplayer 18_ ga _ install.exe", but won't install despite double clicking and right clicking then "open", the egg timer comes on for about 3 seconds then goes off.

Not really, I am trying number 2. but when I click on end process I get a window telling me of doom and gloom if I continue.

You really should get rid of XP, it's no longer supported and the security vulnerabilities and lack of support issues are racking up by the day.Science isn't exact, it's only confidence within limits.0

OK I think it is now installed, from this page https://helpx.adobe.com/flash-player/kb/installation-problems-flash-player-windows.html under heading,"Still having problems", then click and download from Flash Player for Firefox - NPAPI
Now to see if it plays anything.

Above might well be true but at some point both you and the NHS will need to change .
Suggest you run another email client alongside your current one as a trial something universal like Google Mail .0 -
