AI Scheme pre-2005 reclaiming query

Hi, I am one of the many who have received the AI Scheme letter which has a claim form enclosed. My issue is that my 'product' was card protection on a Lloyds credit card from Nov1999-Dec2005. I have read the MSE advice about claiming from the bank as it was sold pre-2005, but I don't have any paperwork/statements relating to the account, it has long been shredded. How would you recommend I proceed?
