do you mean one of the side grills below the bumper line ?
it could fail an MOT IF there are sharp bits sticking out , however an ample covering with tape , or even a cardboard bit stuck over the hole whilst an MOT was taking place would suffice0 -
Only if there are sharp edges which could injure a pedestrian in an impact.

If you do mean that below-bumper grille, then very unlikely - the bumper's plastic, so thick enough to be unlikely to have edges that'd count as sharp, unless the bumper itself's broken in some way.0 -
