I once went for an interview which went really well - they virtually offered me the job on the spot subject to checking references - but then I didn't hear from them for 2 weeks. I then rang the hiring manager to ask what was happening and his response was "We are expecting you to start next Monday. Didn't anyone tell you?"
It turned out that their HR "department" was crap. They were going through a rapid hiring phase and had lost track of me. Fortunately, it all worked out well and eventually they got a better HR manager to sort out their recruitment process.0
