Global Shipping - two nightmares!

Somehow Global Shipping got enabled on a couple of listings - no idea how, as I ship to the EU.
1st issue - Item sent to Austrailia. Has been rejected as 'undeliverable' as the buyer has a PO Box! I do have an 'PO Box' exclusion on all my listings, and can confirm that was present on the listing. Due to 'small print' on Global Shipping regarding PO Boxes, I stand to lose the buyers claim, even though I never had any exposure to the buyers address, and actually exclude PO Box shipping
2nd issue - I sent an item Royal Mail Signed For 1st to the GSP warehouse - the item was never signed for as it common with RM Signed For, but I can see it did arrive there as the Global Tracking shows customs processing and parcel progress. Buyer has opened a Paypal claim as as my domestic tracking doesn't show it got there (even thougb it clearly did!) I stand to lose this Paypal claim.
I have no idea how Global Shipping got 'enabled' as I ship overseas all the time without issues. 2/2 have gone pair-shaped.
