Having problem with getting ex-girlfriend off credit report

Hi there,
Split up with my girlfriend back in 2010.
Equifax keeps showing my ex on my credit report, as having a financial association with her. What's even more frustrating, is that it says Alliance & Leicester, and last updated on 2004.
How do I get this removed?? Have been in touch with Santander, who took over Alliance & Leicester. They can't seem to do anything at their end.
On top of this, my ex is now about to declare herself bankrupt.
She all ready had a dmp in place, which I'm pretty sure is why my credit rating is down to poor now, as I pay all my bills on time each and month.
Any else out there who can offer some advice would be greatly appreciated. Thanks in advance for your comments.

If you no longer have any joint financial products, then file a notice of disassociation with Equifax (and the other CRAs if it appears there).0
