ESA Appeal questions

Hi, I have just had my MR in the post, still giving me 0 points. (very quick actually, only about 5 or 6 working days)
Sorry, lots of questions.
I just want to ask, when the appeal is sent off and they tell the DWP, the ESA will continue to be paid at the assessment rate as long as I still send in fit notes? The man at the JobCentre wasn't sure.
plus, is there any way of getting my benefit that wasn't being paid to me during the MR, I have seen webpages with conflicting information. My fit notes are still valid as of now till the end of this week.
Also, what is the best thing to do when everything I wrote in my fitness questionnaire has changed so much because it took a year from writing it to the actual assessment? The MR and assessment seems to be based on what I wrote last year rather than what I actually said in the assessment, which has changed a lot.
Also, what is the best thing to ask my doctor for for my appeal. I did send him a questionnaire I found on a ESA rights webpage in the post with a letter explaining but the doctors office contacted me and said it would cost £32 for him to complete and I could have a patient record summary instead for free?
