Default showing as current on cc paid off 5ys ago on experian

My experian rating has dropped in the last few days due to a default(red triangle) on a cc paid off 5yrs ago. How do I rectify this.

It also has me at 19 addreses I have only lived at 5.......again how do I rectify this

What is the actual default date? Regardless of when you paid the balance off if it was defaulted it will still appear. When you say '19 addresses' - are they all different? It seems odd.
