Mbna ppi claim

I'm in the process of claiming back ppi (successfully done it myself for loans with halifax and lloyds) and am now trying to sort out the final one with mbna credit card but I have a couple of questions so I can fill out all of the information relevant to my claim.
1) Without wanting to sound stupid ( in my defence it was a long time ago) how do I know / find out if it was an Internet / postal / phone application?
2) At the time I applied for the credit card I was in employment but it was fixed term therefore ppi would have never paid out, however I am now in permanent employment will this impact my claim?
3) I would never have actively chosen to have ppi were mbna one of the companies who pre ticked the boxes on the forms?

I'm after the same answers I too had an mbna credit card back in the 90's and was paying ppi but my memory would not be able to recall whether I ticked or unticked any boxes on any forms

Im also claiming back ppi from mbna, i spoke to them on the phone 3 weeks ago and i had account number etc and i have statements, the guy was quite helpful on the phone, so we will see what happens...
