Help with POC please

I sent capital one a claim for £628 plus 98.18 intersest. They sent me a letter back offering, as a 'matter of goodwill' 148 pounds plus 10 pounds for any interest incurred. sent back an LBA accepting their offer as partial settlement
without prejudice. A letter came back this morning saying they were not prepared to make any further offer and that 158 pounds had been put back into my account, which I have checked and it has.
So now i need to go thru moneyclaim to claim the remaining 568.18 pounds.
Please can somebody tell me how I write out my POC do I just put in it that I am claiming 568.18 pounds, that amount is inclusive of interest or do I somehow have to break it down ?? I dont want to get my claim rejected by making a mistake.
