PPI Claim for my late husband

It has been suggested that I claim back PPI for my late husband, I cannot find the forms on this site to start this off.
I may be being blind & stupid but I really cannot find them.

The executor of the estate is entitled to put in a complaint on behalf of the deceased. However, the complaint reasons are a little more limited as no allegations of what may or may not have been said can be made. Eligibility reasons, suitability reasons (if advised rather than non-advised) etc are perfectly fine to complain about.

the PPI questionnaire is the one to download. You send it to the firm (not the FOS). You will probably need to send a death certificate and evidence that you are the legal representative.I am an Independent Financial Adviser (IFA).
