Documents for 1st child passport

hi all!
before anyone says read the passport office guide lines.... i have lol and im still confused!!
Basically we are taking away my daughters boyfriend with us this year, its his first holiday abroad and will be his first passport.
I know he will need his full birth certificate but what documentation will he need for his parents as
1.... his mum and dad never married and have now split up
2.... he doesnt see his dad anymore and has no contact
3.... his mum has married and has a different surname than him.
so will he need to send off a full birth certificate for his mum and her marriage certificate? will the passport office accept that he has no contact with his father?
