    I'm assuming it happens if people borrow or hire a car that uses fuel that is different from their usual fuel, they then go on auto-pilot when filling up. Or if people have 2 cars, one diesel one petrol, but drive one more than the other.

    I've also heard that some of the petrol stations are not sticking to the standard of black for diesel and green for petrol, especially for the 'high performance' versions of their fuels.

    I used to think that, but as it turns out, really, really easily.

    Case in point: I've owned diesels for five years now. After two years of ownership, I was driving to work one morning when I arrived at the aftermath of a very recent accident on the motorway. I didn't stop, since it seemed like enough people had done so already, and I hadn't witnessed anything.

    Once I drove off, I couldn't shake the feeling that I should have stopped. Drove into motorway services to fill the car up (last resort, since my local station had run out of diesel) still thinking about the accident, and pumped £50 of petrol in. Paid up, drove off and came to a stop on the hard shoulder about six miles later.

    Probably the most expensive mistake I ever made.
