Old provisional licence ~ new address

Hi ~ hope some of you can help.

My partner has a very old (green paper) provisional licence issued in 1988. This was long before any 2 part licences or photo cards.

As he moved address last year for the first time since it was issued and hopes to resume lessons after all this time he needs to change the address on it. The DVLA will want a photo etc to issue a modern one!

We've tried online but as some of the numbers are indecipherable due to it's age and creased with it being folded all this time, we could not proceed online without entering all the numbers.

Do we just fill in the change of address section on the old one and expect the DVLA to issue the correct form to apply/renew with a photo card?
