Filling in the PPI Questionnaire

Hey guys,
Just in the process of filing a PPI claim for my mother, as she never realised she had bought them!
I spoke to Natwest who provided her with three personal loans she paid PPI on, including the start and end dates and their values.
Natwest said to fill in the questionnaire; but i'm not sure who to return the form to?
Also on the questionnaire it only has space for one loan; should I just reprint that page multiple times?

As others have said - but...
Be sure to detail any changes in your mother's circumstances over the years. You should be giving details of your mother's circumstances as per the date of each PPI policy being taken out.0
