Sainsbury's Credit Card PPI - address?

Hi All,

I'm putting in a claim for PPI on my Sainsbury's credit card. I've filled out the form, but looking for the address to send it to. I tried searching the forum and there's two addresses (one for Sainsbury's Bank and one for Card Services) but others also saying that the credit card was underwritten by Bank of Scotland so I should send my claim to them...
Anyone know where I should be sending my claim to?
I took out the card around 2008 if that helps.
I do have an old statement but there's no mention of BoS in the small print.

Finally got my backside in gear to find all the information I needed and send this so want to get it in the post tomorrow if possible.
Any help would be appreciated.
