Maybe you need to get your story straight!
Yes let's get it straight. Initially I wasn't even going to apply for PIP. Then I was convinced that I should so I did. Then I was told to go to a Welfare Rights office which I did and was told that the first appointment was the 17th Feb. The form had to be in by the 5th Feb. So I had to rely on the DWP coming up with the offer they made when I started off the claim - they would help to fill it in. Been there and done that last Monday. Then trying to get some evidence together to send off with the form that contradicts what the DWP put on the form. Realising that the form has to be in their hands by Sunday it meant that it had to be posted by Friday (tomorrow) at the latest. So I had to phone them and they offered 7 extra days with a proviso that the whole lot is posted on Monday without fail.

From what I have read of your posts it seems to me that you have the knowledge and capability to fill out your own forms.
Why rely on someone else who doesn't know you? All the time and energy you have wasted over-thinking and complaining about the process, you could have put together a top notch application.
It really isn't rocket science!

No need at that time as I had another appointment to get the DWP to help me on the 30th.
I had no need to go to the meeting on the 17th and no need to ask for an extension.
What happened since following the meeting this last Monday as I now have a completed PIP2 was to write a letter to go with the form explaining where the DWP have put the wrong info down on the form and getting some evidence together to support that letter. I was running out of time (today) so I had to ring them to explain the situation. They then gave me 7 extra days.0 -

So you can write your own accompanying letter, revise the form as done by a DWP employee and gather evidence to support your claim. Yet you couldn't just do your own form in the first place? Sounds like you're just trying to make a point to me....

I’m doing some research for a person with bipolar 1
The individual has been sectioned three times over the past 4/5 years. They can’t remember the actual dates they were sectioned or when they first got their diagnosis of bipolar. So the first step is to contact the GP and get this important information before making the phone call
The GP is charging £25 for doing the letter and we can live with that but I was wondering what else we could ask him to include in this letter (after all it’s been paid for.
The GP is helpful and understanding so we don’t want to be demanding their records. My friend will have to deal with this GP in future and the Dr might be reluctant to commit himself in the future
Any suggestions

PIP isn't awarded based on a diagnosis, it's how those conditions affect you daily. Doesn't the person have any letters from when they were sectioned, as this would help. Letters from Consultants. When ever i've been to hosiptal to see my Consultant many times in the past i've always recieved the same letter as my GP because they cc them to me as well. If they don't have these then ask the GP if they can have a copy of them. A diary is also good, so i've heard. Take a look at those PIP decriptors and based the evidence alongside the ones that apply.0
