DLA Online Forms

Hi

Last time I had to submit one of these forms, I found the form online. Downloaded it, completed it on my computer and then printed it before it to authorities.

I cannot find DLA1a Adult January 2014 online.

Please can anyone tell me where this is available?

Are you sure that you need to complete one of these?

All new claims have to be for PIP and if it is a renewal I thought they sent you the renewal form?

Hi, I am sure that some-one with more knowledge will help you soon - but as I understand it - anybody over the age of 16 has to apply for PIP now. With the first port of call actually phoning the helpline.

Thanks for the prompt responses.

I am surprised about the form. It arrived in the post today.

The link given is to an older versions, I need January 2014 version.

Could it be because I am already in receipt of DLA, which was granted for 3 years and due for renewal in July this year.

I had a change of circumstance form in October and there was no on-line form anymore. I answered each question on a word document and either did a cut and stick exercise or added as an additional sheet to the paper copy they sent me.
