Halifax Clarity. How do add additional card holder?

No box to tick on application form on Halifax Clarity Card application for additional card Holder. Can you add someone after you have received the card?

My wife obtained a Clarity card via online application a couple of months ago. There was a section on the form to add an additional card holder.
She didn't add me as I already have a Clarity card in my own name.0
