Sentinel CPP - Maiden Name

Hi,
I know there is a thread, but it seems to be discussing and not really any feedback, so not sure if this is the best method of asking the question.
The CPP is through my wife's lloyds tsb card (now tsb).
I was oblivious to this until we received the letter the other week, into the bargain not the greatest on these claim processes.
Questions to ask are:-
1. I am more than certain the card has been running pre-2005, which I believe they will automatically pay out from 2005 onwards (I know it has been cancelled now, not sure when). How do we proceed with making a claim pre-2005?
2. For pre-2005 the wife, wasn't the wife.
Into the bargain I think there was possibly her previous address also, meaning she's been under her maiden name in 2 different addresses. Not sure if this is an issue or should I be right in saying the account number (or whatever) should be sufficient anyway and will tie-in both names?
3. I read that Lloyds didn't sell it, which I think maybe correct, so should I proceed with enquiring about pre-2005 as I am assuming this will have no significance over the recent letter received from AI Scheme?
Hope someone can help, any info not too clear on, please let me know.
