Failed WCA - how to claim JSA and request medical report?

For anyone interested I scored 6 points, which was only because of my seizures. There were also some errors in the ESA85, including the statement that I'm not on medication for my mental health (The same medication that manages my epilepsy also acts as a mood stabiliser). This was information I conveyed to the assessor quite clearly, so casts significant doubt on how well I was listened to and observed.

When i was putting an appeal in i went to our local Welfare Rights, who came with me on the day of the appeal. He told me to get the appeal in Asap, so that you've registered your intent and put a short note in with it saying i was seeing my doctor again to get up-to-date medical information.
I went from 0 points at the medical, to 9 points at the appeal, so unfortunately not enough.
