PIP tribunal - does anyone know where to get the appeal pack?

Hi

My husband has been declined for PIP and we are going to appeal to a tribunal. On the letter from DWP it says we can download an appeal pack from gov.uk/justice.

I dont know if they have been deliberatly unhelpful or if its me not looking properly. But theres no direct link to the appeal pack, even if you search under PIP appeal, benefit appeal etc.
Any help will be gratefully received

Has he done the mandatory reconsideration first?

Read this - link for appeal form is also given:

  • Hi yes he has done the reconsideration. We have also requested a copy of the assessors report and noticed that they had contacted two neurologists but not his GP. The neurologists will only confirm that they are both at different opinons as to the cause and treatment, they wont have an informed opinion about the effects on daily life, We have asked the GP if she will provide a supporting letter - she wont do a letter - she said the DWP have to send her a form which is specific for this purpose. Obviously we are out of the timesscale for this and the DWP wont do this anyway as they have made their decision.

    Thanks for the link.
