Form ESA 3 Please please help

hi I have to re-apply for my esa as my contribution based esa comes to an end at the end of Febuary. I have been given a esa 3 form to fill in, but I have hit some snags

Part 5 the about work section
1) I did do some self employed permitted work that the DWP know about as an IT consultant. Will I have to fill in a B16 self employed form or is the fact it is permitted work exempt me from that

2) after asking questions about your work it asks "do you know when you will be well enough to work again" and "are you going back to work" do you have to answer these at all.? If you put yes you have to provide a date. Can these questions be left blank?

  • I would put in the self-employed work and then near the back you have a chance to add anything else. I would put in there that the work was permitted work, maybe add a bit near Part 5 also that it was permitted work.

I am guessing the question about knowing when you will be well enough to return to work is more geared towards people that have had an operation or treatment with a likely end date. I would just tick No unless of course you know when you will be well enough.

For the first question I would simply note that the only work you have done is permitted work, previously declared.
I don't understand your problem with the second question. If you don't know when you will be well enough to work the answer is "No". As you are not returning to work at the moment the answer to the second part is also "No".
