medical exepmtion certificate

does anyone know about medical exemption certificates?
I got sent a letter from my hospital asking me to sign it and send it off, and I assumed i'd then receive an exemption certificate in the post?
But this was about 3 weeks ago now. Maybe it got lost? Or maybe it takes longer than the '10 days' that I read it takes?

    Perhaps it was meant to be in with the letter-ring them and ask.

    Do you mean they sent you a form to apply for a certificate?

    Had the hospital doctor signed it or did you get your GP to sign it before sending it?

    Are you sure it was an medical exemption form..FP92A?
    Usually it would be your GP who would have to complete and sign along with your signature. I received my card within a week of signing the form and leaving it with the surgery for the GP to complete and post....
    You need to chase it up.

    Hi. Yes, it was a little form signed by the hospital Doctor. She had filled out my information, and told me to sign it and send it off.
    Nothing heard since.

    Use the number Sheramber has quoted and chase it up.....Its not usual for a hospital doctor to complete these forms...and to be honest I didn't even know they held them as its GP issue form...and you have waited over the usual amount of time...Medical exemption cards are normally sent out on receipt of the form and you should of received it at the very most 10 days after the form was posted...

    Ring them and check they have the form...if not speak to your GP and if you fit the criteria he/she will complete another. Or it could be a case of lost post.

    The green and blue form FP92A I finally am in receipt of, has "GP's or hospital's doctors stamp area" on sign off at part 3 doctor's statement which says "this form can be signed by a GP, hospital or service doctor" so it clearly isn't exclusive to GP's

    A hospital can access a patient's records I would have thought just as it say's, though there seems to be some fetish of only writing in black ink and block capitals acceptable on form (so perhaps this is causing delay)

    Don't make it easy do they OP,
    next I hope my GP surgery doesn't try and charge me an admin fee for trying to obtain the doctor's signature on this blooming form, though wouldn't surprise me, I am getting sick of going round in circles over this form! must perserve and unfortunately nowhere on the form does it give any guidance on when to expect to receive the card/certificate

    Just checked an I apologise the forms now do extend it too other relevant medical professionals. I remember the forms where revised about 5-6 years ago so the change may of taken place then. The hospital won't have access to any medical information the GP holds on a patient unless a summary is requested..

    keepcalm.....Your GP can't charge you for the completion of this form as it come under NHS remit.
