Budget Plan Template

I've just watched Martin's video called "Debt Problems: Where to start & what to do." to start me off on my journey of getting myself debt free. Like another users story I read - I have been gradually digging myself further into debt since my early 20s. I'm fast approaching 30 and I don't want to go through my 30's on my debt-filled path!
He mentions in the video that you need to start with a budget plan, knowing what you earn, what you spend, where etc. I thought from that there would be an easy to find template but so far I haven't found it :-( Has Martin posted it somewhere and I'm just missed it?
I really want to do this and then submit the results on the Forum so I can get some proper help.
