I don't think anyone gets the actual certificate from their GP or midwife, the form maybe but that has to be sent away. The certificate is plastic and personalised and takes a couple of weeks to arrive.0

I have tried at least 3 times to get the form - to no avail. I was last told, when I took my baby to be weighed at my GP's surgery (he's now 5 1/2 months old) that nobody checks for the certificate and they have never known anyone to be chased if they don't have one so I should just tick the box to say that I have a certificate. I have asked two 2 midwives about getting the form and a health visitor. I also mentioned not having the certificate to my GP - she told me not to pay as my proof is the baby!
I haven't paid for my last two prescriptions - I am taking anti-depressants. I was paying before that because I didn't have the certificate but because of the advice given I stopped paying.
What I'm a meant to do? No one will give me the form I need so really I should be paying but the people who should be giving me the form won't/can't give me one! So even though I should be exempt from paying I should have to pay because I can't get a form. It is a crazy situation. Especially when I have post-natal depression, the condition I am suffering is related to my pregnancy.
I don't know what to do, should I start paying again to avoid a fine even though I have been told not to pay???0 -
Ask the secretary at your GPs surgery, that's who has them at mine.0

Maybe the board guide can move this to the correct board...
I have no idea why they would refuse to give you the Maternity exemption form...You need to speak to the Practice Manager at your surgery.. failing that your Health Visitor should be able to speak to the Practice.
The maternity card runs out 12 months after the babies birth or if the babies a premium, 12 months after the expected date.
You need to pay for your prescriptions in the meantime... Get the form send it off , save your receipts for any paid for after the form is dated and sent...You will be able to claim your money back .0 -
The health visitor should be able to issue the form too, according to the NHS website.0
