Giving Notice to a Tenant to End Tenancy

We have a BTL which has had the same tenant in for about 14 years. We now need the property back for own personal use due to change in circumstances. Originally a 6 month AST which is now rolling one month/ periodic. Tenant ok with going but requires official notice to give to local Authority to get on list for replacement property (pensioner, registered disabled). Ideally we want to give 1 month's notice as it's now on a rolling 1 month basis, if possible.
Can anyone point me in the right direction of where I can find a form for it, please? have been googling but just ended up confusing myself

I'm afraid you wont be able to evict within one month unless you had the tenants agreement, but that seems unlikely if the tenant wants to be rehoused by the council.*Assuming you're in England or Wales.0 -
You will need to serve a two months notice at the end of the rolling month.
This is called a Section 21, they are very easy to !!!!!! up so it might be better using one of the online services.I do Contracts, all day every day.0 -
And you will have to wait until the S21 notice is up (two months) then get an eviction order from court and possibly appoint bailiffs if the council insist on this before they will rehouse your tenants.
It will definitely take two months to have the tenants leave, could be four.0 -
